Volunteer Coach Application Form
Would you like to volunteer to teach sports skills to children in the community? Please fill out the volunteer form and submit back for us to keep on file when your choice of sport comes up.
Starting new in 2012, all volunteer coaches for youth sports will be required to take the Coaching Certification class through the Pueblo West Parks and Recreation and the new state law "Concussion in Sports" free online course through the National Federation of State High School Associations (register here):

Upon completion of the courses, coaches will receive a certificate to print (or download as a PDF via email) and will need to return it to the Pueblo West Parks and Rec office as proof of passing the course. (You may alternatively submit your PDF of the certificate at the bottom of the form below.)
These new changes are required by all recreation departments and youth sporting activities.

All coaches follow the requirements of yearly background checks and mandatory sports clinics competed at the Pueblo West Parks and Recreation office.
